All registrations must be submitted/processed at the Students Service Department

Open Monday - Friday 8:00 am to 4:00 p.m. (closes for lunch from 12:30pm to 1:30pm)

Requirements:

• Registration Forms
• Child's Birth Certificate
• Physical Exam
• Immunization: • 4 Polio   • 5 DTP (6 doses 7th & 8th Grade)  • 2 MMR  • 3 Hep B  • 1 Varicella
  
• Current TB Test (within 12 months)
• Proof of address

All steps in this process must be completed before a student can be enrolled in school.

Children who will be age 5 by November 2, 2012 are eligible to attend kindergarten.

You will receive a confirmation of enrollment notification from the Students Service Department by mail in July. You will then receive follow up correspondence from your assigned school by August.
